How to Join
Kairos is formed under ARS § 11-952.01, so you are not required to undergo the competitive procurement process in order to join.
Joining Kairos starts with us providing you a quote for your employee benefits program. The information we’ll need is detailed below (“Census,” “History,” etc.), or you can download an info sheet here.
If you decide to accept your quote and become a Kairos member, staff will work with you to ensure a smooth, speedy, seamless transition.

Employee census (date of birth, gender, home ZIP code, plan election, and tier selection, preferably in Excel)
Minimum of most recent 12 months’ claims experience, broken out by:
• medical claims
• pharmacy claims
Large claim report for the corresponding time frame, including:
• total claim amounts over $50,000
• primary diagnosis
• active/termed/COBRA status
Current specific stop loss rate and stop loss deductible
Plan design benefit summaries
Rate history (two years, if available)
